REGISTRATION PAPERWORK COLLECTION
This year our registration paperwork will be turned over to us in person. This will take place on the same evenings we are doing the uniform fittings.
The dates are as follows: 5/15, 5/17 and 5/18.
Registration Paperwork needed:
- Proof of grade (print report card off Infinite Campus, you can blackout letter grades, we just need name and grade level.
No grade proof needed for kids entering K) - Copy of Birth Certificate (if you were part of the program last year we have this and do not need another copy)
- Physical dated after 8/1/22, we need it to say the athlete is cleared to participate in activities.
(if your physical is in the summer, please turn in the physical at the parents meeting on 7/31, without the physical your child can not practice on 8/1) - Participant Contract

- Parent Responsibilities
Please bring with you all the paperwork for the registrar on the date you sign up for.
